Authorization and General Terms. I (the use of ?I? refers to Account Holder and Joint Account Holder, if applicable, jointly and severally) authorize Foursight or its affiliates with whom I have an account (collectively, ?Foursight?) to automatically deduct the scheduled monthly payment and the other amounts described below, if any, each month from the checking or savings account that I have provided on the scheduled monthly payment due date as indicated on my Contract (the ?Due Date?). I authorize the financial institution holding that checking or savings account to honor the deductions. I understand that these checking or savings account deductions will be applied as monthly payments under my retail installment contract or promissory note, as applicable (my ?Contract?).
If my Due Date falls on the 29th through the 31st of the month, Auto-Pay will withdraw the payment on the last day of the month if the current month has less than the number of days needed to reach the scheduled Due Date. This authorization will remain in effect until Foursight has received notification from me, at least three (3) business days before the next scheduled automatic fund transfer date, stating my intent to terminate ongoing Auto-Pay payments. I understand that Auto-Pay may not automatically terminate even after the last Scheduled Payment under the Contract is made. Thus, I agree to promptly provide Foursight with such notice of Auto-Pay termination if I pay off the Contract early, if the Contract no longer has any payments due, or if I no longer wish to have my Scheduled Payments paid through Auto-Pay.
Fees and Past Due Payments; Paid Ahead Payments. I understand and agree that the monthly and recurring deduction will be for the scheduled monthly payment amount as indicated on my Contract (the ?Scheduled Payment?). I further agree that if any fees accrue under the Contract (such as late fees or return item fees) or if any past due partial or full installments accrue under my Contract, that Auto-Pay will not automatically withdraw those amounts and that I am responsible for paying those additional amounts to Foursight under the terms of my Contract outside of Auto-Pay. I further understand and agree that even if my Contract is paid ahead because of prior amounts applied to future payments not yet due, that Auto-Pay will not take into account the paid ahead status and will continue to deduct the Scheduled Payment each month. Notice. Foursight will notify me of the actual amount deducted in the following month?s billing statement and by posting the total amount deducted on my online account record on Foursight?s website. I understand that I must verify the checking and savings account information in addition to the bank routing number with my financial institution before submitting this enrollment and authorization to Foursight. I agree to indemnify and hold Foursight and its affiliated entities harmless from any damage or claims arising from my provision of inaccurate information that may result in incorrect payment deductions or late or missed payments. I will continue to make my Scheduled Payments by the Due Date until my first Auto-Pay payment is completed. I understand that Foursight must receive this request at least ten (10) business days before my next Due Date, otherwise, Auto-Pay may not activate until the next Due Date.
